Wellness Plan

Adult Wellness Plan $350

(per 12 month period)

Healthy Adult**

  • 2 healthy mouth cleanings
  • 2 check-up exams, including oral cancer screenings
  • 1 emergency exam
  • All necessary radiographs
  • 15% off regular office fees on all dental treatment

**This is a discount offering and not considered dental insurance coverage and cannot be used in conjunction with dental insurance.  Does not apply to Sonicares, rinses, tongue cleaners or any other dental products sold in our practice.

**Ineligible if periodontal disease is present

Child Wellness Plan**

$199 for 1st child;
$149 each additional child

(per 12 month period)

  • All necessary radiographs
  • 15% off regular office fees on all dental treatment

To determine eligibility, the comprehensive examination with a full set of x-rays- $149

care-credit

Care Credit is a third party financing system. When you sign up with care credit, your payment is paid in full by Care credit at the time of service and you will need to pay back Care Credit in a fixed number of months. Depending on the amount charged, the repayment options vary. Contact our office today if you have questions or would like to make an appointment.
